Patient Rights & Responsibilities
Health care is a shared experience involving patients and those who give care. The personal worth and dignity of each patient is recognized at Fayette County Memorial Hospital. Your Rights and Responsibilities are offered as an expression of our philosophy and commitment to you.
You Have the Right.....
To be treated courteously with dignity and respect and in confidence by all medical staff and hospital personnel.
As a patient, not to be discriminated against due to race, religion, color, sex, age, national origin, military status, ancestry, disability or source of payment for care.
To have access to interpretative services as needed.
To have your personal privacy and safety maintained and to be free of all forms of abuse and harassment.
To receive complete and accurate information concerning your diagnosis and treatment in terms that can be understood.
To have any proposed procedure or treatment explained to you, and to have information necessary to enable you to make appropriate decisions.
To know the identity of physicians, nurses and others involved in your care.
You have the right to actively participate in the development and implementation with your physician and other healthcare providers in planning your treatment.
To accept or refuse any procedure, drug, test or treatment and to be informed of the possible consequences of any such decision.
To have or formulate an Advance Directive, which may include a Durable Power of Attorney for Health Care to the extent permitted by law and hospital policy.
To expect that all information and records about your care will be kept confidential, except in such cases such as suspected abuse and public health hazards when reporting is permitted or required by law.
To receive a notice describing how medical information about you may be used and disclosed.
To request restrictions on definite issues and disclosures of your Private Health Information.
Access information contained in your medical record within a reasonable time frame.
To every consideration of privacy.
To be free of restraints of any form unless medically necessary.
To be informed of research or education projects affecting your care and treatment.
To pain management, including assessment and education on pain management.
To participate in planning for care after discharge.
To receive, upon request, a copy of a reasonably clear and understandable, itemized bill in a timely manner, and upon request, to have charges explained.
You may receive information on how to express a complaint or grievance concerning the quality of your care by calling 333-2950 and also to appeal to an external agency.
You may seek review by the QIO for quality of care issues, coverage decisions and to appeal a premature discharge. Please see telephone numbers on back.
Your Responsibilities are.....
To provide information, or clarification if needed, about past illnesses, hospitalizations, medications and other matters related to health status.
To ask for pain relief when pain first begins, and to tell your nurse or doctor if pain is not relieved.
To ask questions when you do not understand what you have been told about your care or what is expected of you.
To provide a copy of your written Advance Directives, if you have one.
To inform your physician(s) and other caregivers if you anticipate problems in following the prescribed treatment.
To provide necessary information for insurance claims and for working with the hospital to make payment arrangements, when necessary.
To follow the rules and regulations concerning patient care and conduct and to be considerate of the Fayette County Memorial Hospital staff and property.
To recognize the impact of your life-style on your personal health and accept the consequences for the outcomes if you do not follow the care, service or treatment plan.
