Patient-centered medical home is a care delivery model focused on you, the patient, designed to improve your health and well-being through coordinated care and accessible services using a physician lead Care Team.
What is a Care Team?
The Care Team is you, your chosen personal physician, and the individuals that deliver and support coordinated care. This team helps guide you to meet your health care goals now and in the future.
Who is part of my Care Team?
You, your trusted physician, nurse practitioners and physicians assistants, clinical staff (medical assistants, licensed practical nurses, and registered nurses), and the receptionist/front office staff. Nurse practitioners and physicians assistants are highly trained individuals who work directly with your doctor to improve access and meet your needs.

How can I contact my Care Team when the office is closed?
You can call (740) 333-3333 after the office is closed to ask a medical question or leave an urgent message for the on-call provider. For questions that can be answered on the following business day, you may send a web message using the patient portal. For emergencies, please call 911.
What do I do if I am sick and need an appointment?
You can call (740) 333-3333 during normal business hours to make an appointment with your Care Team. We also offer same day walk-in appointments with the Same Day Care Team.
What is the patient portal?
The patient portal is a secure internet based service where you can request an appointment, pay your bill, review your medical information and contact your Care Team. Your physician can communicate directly with you using the portal.
How can I sign up for a patient portal account?
You can call (740) 333-3333 and speak with a receptionist/front desk staff member or you can provide your email address at the time of registration.
Where do I access the patient portal?
Simply go to the hospital website at www.fcmh.org and look for the “Physician Practices” link. This will take you to our homepage where you will find the link to the patient portal.
We provide equal access to all of our patients regardless of source of payment.
