1. How do I obtain copies of my records?

In order to protect your privacy, you must sign a release form and provide photo identification.

2. How much notice must I give?

A 24-hour notice is appreciated. You may call ahead and we will have them ready for you when you arrive to sign your release form.

3. Is there a cost?

There is a charge for the service of copying your records. You may call for the exact cost. Our charges are according to the state-required minimum.

4. Do I have to provide a photo ID?

Yes

5. How do I request copies of my radiology films?

You may call directly to the Radiology department or call Medical Records. A 24-hour notice and photo ID are required for this service.
