Employee Benefits Overview
Plan Year: January 2008 - December 2008
1. Ohio Public Employee Retirement System (OPERS)
The hospital is a political subdivision (county facility) of the State of Ohio, all employees are automatically enrolled in the State funded/supported OPERS retirement program. OPERS is in lieu of social security (except 1.45 % Medicare Tax); and, it requires contributions of 10.0% of gross pay for all employees while the hospital is required to contribute an additional 14.00% of this gross pay amount for all employees. Should an employee terminate employment, for any reason-prior to actual retirement, the employee may withdraw only those funds, which have been directly contributed by the employee. Neither the employee nor the hospital is eligible for a refund /return of any hospital contributions which have been made to/for this retirement fund. A full month's service is given when an employee grosses $250.00. If you earn less than $250.00 in a month, you will receive part-time service credit. Some of the benefits that OPERS may offer include:
- Retirement-You may receive a retirement benefit beginning at age 55 if you have 25 years of credited service or at age 60 if you have 5 years of credited service.
- Survivor Benefits-After 18 months of credited service, your dependent children and surviving spouse would be eligible to receive a percentage of your final average salary.
- Disability-After 5 years of credited service, coverage is provided for permanent disabling conditions that prevent you from performing your job.
- Retiree Insurance Coverage-With 10 years of credited service, you and your eligible dependents qualify for the OPERS health care plan. Dental, vision and long-term care protection is also available.
- Any public service from other entities will be evaluated for years of service for vacation accrual purposes only. This will base on verification from employees previous employers.
2. Health Insurance
All full and part time employees are eligible to participate in the group medical, dental, vision, and prescription drug insurance program offered by the hospital. Health insurance is available on an optional pre-tax basis and employees can enroll in these products annually during open enrollment or at the time of hire. There are shared co-payment amounts via payroll deduction for these premium costs. Health insurance is effective the date of hire. There are ten (10) days to submit enrollment forms. 3. Employee Health Forgiveness Program
Full-time or part-time employee and members of their immediate family who are participants in a qualified health program are eligible for this discount. (Immediate family includes those dependents who are under the age of 19 or up to the age of 25 if enrolled as a full time college student). Covered services will include only services which are specifically provided by the Fayette County Memorial Hospital. Discounts will be computed only on the 'remaining balance' of the patient's bill after the insurance company has determined and paid its share of the submitted claim. The discount will apply only to coinsurance payment responsibility for the eligible member after the deductible has been met.
4. FSA
Fayette County Memorial Hospital offers a Flexible Spending Account to full time and part time employees. There is $2,000 maximum pre-tax contribution per year for each employee. The FSA election amount will be divided into 26 equal amounts or remaining pay periods, to be deducted every payroll. Employees that sign up for a FSA will receive a Flex debit card for the election amount. The Flex debit card will expire on December 31 every year or upon termination.
5. Vacation Time
Full-Time and Part-Time employees shall accrue vacation time benefits based upon the following schedule:

The actual number of vacation hours accrued will be dependent upon the actual number of hours worked throughout the year; and, the actual accrual for any given year of employment may not exceed the accrual rates noted. Additionally, the carry-over of unused vacation time from one year to the next-may not exceed two (2) years of the accrual rate for an employee.
6. Paid Holidays
All Full-Time employees are eligible for ten (10) paid Holidays per year-dependant upon the actual hire date of the employee. The following dates are recognized as Holidays by the Hospital:

All "hourly" employees who work on the dates marked with an asterisk (**) will be paid an overtime rate which represents a (50%) premium payment-above their normal rate of pay for actual work performed on these dates. (For ease of calculator, payment for these worked Holidays may be calculated by multiplying the normal base rate of pay times a factor of 1.5). The overtime premium rate is not available for the floating Holidays or for those who do not actually work the indicated Holidays.
It should be further noted that the 11PM-7AM shifts, which occurs on the evening before the holiday, shall be utilized for determining the holiday premium payment rate for those night shift employees who are eligible for this overtime premium.
Martin Luther King Day, President's Day, Columbus Day, and Veteran's Day are considered "Floating" Holidays. "Floating" Holidays are holidays that may be taken on days other than the day in which they specifically fall in the calendar. A floating holiday must be scheduled in advance and approved at the discretion of the supervisor/manager in consideration with the needs of the department. Floating holidays can not be carried forward to the next calendar year and need to be used by the pay period ending 1/3/2009. This will enable employees to use their "Floating" holiday through Christmas providing it is approved by their supervisor/manager. Employees can not be paid in lieu of taking floating holidays.
7. Sick Time
Sick leave benefit time will be accumulated by all part-time and full-time employees at an accrual rate of .0576, which is multiplied times the number of actual hours that the employee works per year. There is no overall cumulative limit of the accrual amount.
Appropriate use of sick time benefits are spelled out in the employee handbook and the use of these benefits may be affected by federal legislation (i.e. Family Medical Leave Act). Also, since the sick time allowance is not a vested benefit, all such sick time accruals are rescinded upon termination of employment with the exception of the special buyback provision of certain hours for those employees meeting the established qualifications for formal retirement from the hospital.

9. Educational Assistance
The Hospital offers 90% education assistance to all employees up to a maximum of $5,250 per year. A request for educational assistance must be completed and approved prior to class registration.
The Education Assistance program offers payment toward educational tuition that was incurred; are related to an employee's job position; and have been previously approved by an employee's manager as well as the Vice President of Human Resource. Actual reimbursement is contingent upon successful completion of the course.
10. PRN Benefit
In lieu of the vacation, sick, holiday and the majority of voluntary benefits (with the exception of the mandatory/regulatory OPERS retirement program) all PRN employees are paid a premium wage rate during employment. This premium rate represents a fifteen (15%) percent additional differential, which is added to the employee's base rate of pay. The base rate of pay, for all PRN employees, is established in accord with the normal wage and salary scales for the hospital.
11. AFLAC - Supplemental Insurance
Full and part time employees are eligible for supplemental insurance policies through AFLAC via payroll deduction. These products are available on an optional pre-tax basis and employees can enroll in these products annually during open enrollment or at the time of hire. Supplemental insurance plans available through AFLAC include: Short Term Disability, Intensive Care, Accidental, Personal Recovery Plus (heart), and Cancer. An AFLAC representative will meet with you one on one to discuss our Cafeteria Plan (pre-tax benefits) and the products noted above. This benefit is effective first of the month following date of hire. In the event that an employee is on an unpaid leave of absence, this benefit will be billed to the employee's home address to pay for directly until returning to work, at which time the payroll deduction(s) will be re-activated.
12. Life Insurance
Basic Life and AD & D Insurance -
All full time employees receive group life insurance and accidental death and dismemberment (AD & D) benefit paid for 100% by the hospital. The following is the employee benefit based on annual salary.

Class 2: Earnings over $50,000 1X annual earnings rounded to the next higher $1,000 Maximum of $175,000.00
Voluntary Life and AD & D Insurance -
Full time employees are eligible for the voluntary group life plan and accidental death and dismemberment (AD&D) benefit through payroll deduction. Unum Provident Company offers these products for the employee, spouse and children in various benefit increments. Cancellations to this benefit are permitted only at the time of open enrollment. As well, late entrants will be accepted only during open enrollment, but must prove medical insurability at the employee's expense. In the event that an employee is on an unpaid leave of absence this benefit will terminate and the employee will be eligible to re-enroll upon date of return.
13. Employee Assistance Program (EAP)
A strictly confidential counseling service provided 100% by the hospital for full time and part-time employees. The EAP provider is Mt. Carmel Behavioral Healthcare, offering counseling services twenty-four (24) hours a day through a toll-free number on various issues including (but not limited to) alcohol/drug use, anxiety/depression, eating disorders, family relationships, financial concerns, job stress, marital, parenting etc. Each employee and any employee's family members (living within the household) are eligible for eight (8) free counseling sessions for each issue per year, if counseling is deemed necessary.
14. COESRA -(Central Ohio Employee Services and Recreation Association)
ESM - (Employee Services Management)
As an employee of the hospital, you automatically become a COESRA member. Employees have access to discounted products and services at various organizations throughout central Ohio. Please refer to the 2007 COESRA discount flyer for a complete list of locations offering discounts. All employees (including PRN) are eligible for this benefit.
15. Pre-Paid Legal & Identity Theft Services
Pre-Paid Legal Services, Inc. and Subsidiaries offers a valuable benefit by providing much needed relief from the financial burdens and emotional stress that usually accompanies legal problems. PPL provides support on a variety of legal issues or situations including (but not limited to) writing/revising your Will, ex-spouse issues, traffic violations, purchasing a home new or used car, advice on community property laws, credit issues and many more. PPL has an additional benefit option called the Identity Theft Shield that offers credit reports, continuous credit monitoring, identity restoration and identity restoration reimbursement. Both benefits are available to full and part time employees via payroll deduction. Enrollment or cancellation to this benefit may be done during the annual open enrollment period (April) or at date time of hire. In the event that an employee is on an unpaid leave of absence this benefit will terminate and the employee will be eligible to re-enroll upon return effective first of the month following return to work date.
16. Ohio Deferred Compensation
Full and part time employees are eligible for Ohio Deferred Compensation, a voluntary, supplemental retirement benefit plan regulated by section 457 of the IRS. Ohio Deferred Compensation is available to public service employees (OPERS employees). Employees may enroll anytime during the year through the Ohio Deferred Compensation system choosing Plan features and investment options on a pre-tax basis through payroll deduction.
17. Court House Fitness
Full and part time employees are eligible for this a discounted membership rate to Court House Fitness, a local fitness club in Washington CH. Employees may enroll anytime during the year by completing a Court House Fitness payroll authorization/deduction form. Some of the fitness services offered include: an aquatic center, 24-hour access to cardio and weight area, tanning, ladies loft, and many more!
18. Fayette County YMCA Corporate Membership Program
All full and part time employees will have access to a state-of-the-art fitness center, on staff personal trainer and many more amenities throught he YMCA Corporate Membership Program. Fayette County Memorial Hospital will receive a 10% discount on individual and family rates.
